Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga.  The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. Position Summary:

 

PCH is seeking a Social Worker to support the Partners Community Health team.  Reporting to the Director of Care, the Social Worker collaborates with the inter-professional team to assess and develop a plan of care that meets resident psychosocial and social needs promoting the resident overall wellbeing and safety.

Key Responsibilities:

 

  • Works in collaboration with other team members, such as Associate Director of Care, Registered Practical Nurses, Registered Nurses, and allied health team members to deliver resident care based on assessed needs
  • Coordinates residents move in, transfers and move out process to include facilitation of the application process, tours, introducing the resident/family/SDM to the care community and its operations, assists and supports resident/family/SDM with adjusting to living in long term care
  • Plan, develop, implement, and evaluate individual social work services and/or support groups to meet the identified needs of residents and families
  • Engage external resources as required to ensure resident social needs are met
  • Act as a support and an advocate for residents’ rights in areas such as independence and control over decision-making, managing and/or maintaining social and family connections and conflict resolution
  • Follows up on concerns/complaints that arise in the resident home area(s)/neighbourhood(s) to ensure that they are addressed in a timely manner and achieve satisfactory outcomes
  • Supports residents and families during advanced care planning and treatment plan conversations within Social Worker/Social Service Worker scope including funeral arrangements discussions
  • Works along with the Manager, Office Administration to ensure that financial documented is completed upon move-in and provide ongoing assistance to access financial resources as required i.e., rate reductions, Canada Pension Plan, Public Guardian & Trustee
  • Coordinates and facilitates resident care conferences
  • Contributes to resident/family survey results to improve satisfaction with care
  • Act as a resource and support by educating team members on best practices
  • Assist with the provision of a resident advocacy program through the promotion and adherence of the Bill of Rights
  • Coordinates and facilitates Family Council
  • Facilitates resident/family support groups within the care community
  • Chairs Ethics/Palliative Care Committees meetings and participates in other care community teams as required
  • Provide necessary and up to date education on community resources and/or processes as needed for staff and family/residents
  • Liaise with community resources/agencies to keep informed and facilitate community partnerships
  • Recruit and supervise social work students’ placements
  • Works in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act/Work Safe BC
  • Performs other duties as assigned

 

Qualifications: 

 

  • Bachelor of Social Worker from an accredited institution
  • Registered Social Worker as per provincial requirements and license in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) in healthcare or social services
  • Minimum of three years’ experience in healthcare or social services
  • Experience in the field of Gerontology preferred
  • Familiarity with the multi-disciplinary team approach, teamwork skills, and the ability to function effectively in a collaborative environment
  • Customer service-related experience an asset
  • Strong oral and written communication skills required
  • Ability to organize, plan, and delegate responsibilities
  • Willingness to upgrade and maintain personal and educational qualifications as required by the changing needs of the role
