Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. Completed in 2023, the new homes have a combined 632-beds and are part of PCH’s larger strategy that will introduce innovative and inclusive programs and services and new models of care delivery. Position Summary:

 

PCH is seeking an Activation Aide to assist the Behavioural Support Unit (BSU) at Partners Community Health, a transitional home area that provides specialized care for people with responsive behaviours associated with a dementia and older Adults with Mental Health Disorders. As an Activation Aide, you will be responsible for planning and implementing engaging recreational and therapeutic activities for our residents. Your creative and compassionate approach will contribute to the physical, emotional, and social well-being of those in our care.

Key Responsibilities:

 

  • Coordinating, planning activities, communicating programs, purchasing supplies, decorating, setting up, facilitates, overseeing, and assisting residents, cleaning up, documenting, and evaluating programs.
  • Actively and effectively facilitating programs in the care community including but not limited to, small and large groups and special events that meet the needs of the residents.
  • Completing resident assessments and establishing goals and interventions based on the residents’ identified needs and concerns.
  • Completing documentation related to programming including but not limited to assessment, attendance, progress notes, care plans, and quarterly reviews.
  • Seeking opportunities to collaborate with other therapy programs i.e., pet therapy, music therapy, art therapy, and to connect the long-term Care residents with the community.
  • Promote a positive and inclusive environment that encourages resident participation in activities.
  • Provide support and assistance to residents during activities, ensuring their safety and enjoyment.
  • Collaborate with the interdisciplinary team to develop and implement personalized care plans.
  • Performing other duties as assigned.

 

Qualifications: 

 

  • Post-secondary diploma or equivalent in activation, recreation, and leisure studies, therapeutic recreation, kinesiology or other related field from a community college or university or current enrollment in a community college or university in a diploma or degree program in such a field.
  • Familiarity with the Gentle Persuasive Approaches (GPA) program is considered a plus
  • Preferred candidates will have completed U-First training, showcasing their expertise in implementing person-centered care approaches for individuals with dementia
  • Applied Suicide Intervention Skills Training (ASIST) certification or Crisis Intervention training would be considered an asset
  • Current Standard and/or Emergency First Aid and CPR certification.
  • Food Handlers Certificate is required.
  • Previous experience in program planning, documentation, and evaluation required.
  • Competency in Point Click Care, MS Office an asset.
  • Possesses strong interpersonal and communication skills.
  • Ability to work independently and as part of a collaborative team.
  • Knowledge of regulatory requirements and best practices in Long Term Healthcare activities.
  • All applicants must successfully pass the prescribed Vulnerable Sector Check.
