Nurse Practitioner-Episodic Access to Virtual Care

2 FTE Positions
Thunder Bay Site

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking two (2) FTE Nurse Practitioners (RN-EC), who as valuable members of the Community Health Centre team will provide virtual episodic access to health care to clients with low acuity health concerns. The Nurse Practitioner will activate pathways if required referring clients for further consultation and assessments, wrap around supports, and will connect them with services in their community within the NorthWest Region.

The position has a competitive salary and the Health Care of Ontario Pension Plan.

Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment. Click here for eligibility criteria

QUALIFICATIONS:

  • 3-5 Years Experience as a Nurse Practitioner, with experience providing virtual care.
  • BScN in nursing from a recognized University and a Certificate in the Extended Class from CNO.
  • Three to five years work experience in a combination of remote rural, hospital and community settings and/or public health along with experience as a Nurse Practitioner providing care to individuals who face multiple barriers to care.
  • Prescribing Narcotics and Controlled Substances certificate from a CNO-approved training program preferred.
  • Awareness and sensitivity to socio-economic and cultural needs.
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
  • Demonstrated commitment to and knowledge of community-based health care.
  • Flexibility in work hours is required including evenings and weekends.
  • Awareness of northern community health care needs.
  • Demonstrated critical thinking and problem-solving skills, organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team environment.
  • Demonstrated ability to effectively manage care needs in a professional manner.
  • Superior communication (verbal and written), interpersonal, and conflict resolution skills.
  • Proficiency with Microsoft Office applications (e.g., Word).
  • Valid Ontario Driver's license and access to a reliable vehicle.
