Registered Practical Nurse 

1 FTE Contract until March 31, 2025

One of a network provincial community health centres, the NorWest Community Health Centres is acommunity based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking 1 RPN, as part of the interprofessional care team, to use the principles of client centred care within the context of self-directed and interdisciplinary team approach, providing clinical care, illness prevention and health promotion within a community health centre environment. The focus will be on providing health care services to newcomers and individuals at risk who are residing in areas where individuals are experiencing barriers to care. This is a new funded position with the possibility of a contract extension.

This position has a competitive salary and the Health Care of Ontario Pension Plan.

QUALIFICATIONS:

• RPN with current College of Nurses of Ontario registration.
• Minimum 3-5 years nursing experience.
• Ability to work flexible hours including evenings and weekends.
• Strong organizational, communication and leadership skills.
• Ability to work both independently, and collaboratively, within interdisciplinary team environment.
• Perform clinical care according to the College of Nurses standards for nursing practice.
• Utilizes professional assessment skills to identify physical, mental, and psychosocial needs of clients.
• Experience in supporting new comers and individuals facing barriers to health and social services.
• Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction.
• Proficiency in electronic charting.
• Has awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
• Valid Ontario driver’s license and access to a vehicle.
