ROLE PURPOSE

This position will provide support to residents of the Connective Support Society's programs in areas relating to individual risk and needs in order to ease their transition to the community and assist in their reintegration efforts, ensuring the orderly and safe operation of the facility, and maintaining the goals of the program. This position is responsible for the provision and coordination of services to residents in the community.

STATUS
This position is non-unionized. Open to applicants of all genders.

HOURS (40 HOURS PER WEEK)

 

Monday to Friday, 1000-1800


This is a hybrid role that is primarily outreach-based. There will be times that the worker will need to work at SHARP and therefore will need the ability to get reliability security clearance.


KEY ACCOUNTABILITIES

  • Works with the client, researches background material, and seeks the input of other professionals and family members, to assess needs and intervention possibilities
  • Works with the client to consider issues and agree on an intervention and support plan with measurable short and long-term goals
  • Ensures the client’s intervention and support plan addresses issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure, lifestyle, access, employment, and education
  • Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals


EXPERIENCE AND SKILLS

  • A Diploma in Social Services and one-year relevant experience is preferred; inhouse training can be provided for the right candidate
  • Professional or lived experience working with vulnerable populations experiencing multiple barriers
  • A demonstrated and clear ability to respond well in crisis situations

REQUIREMENTS

  • Ability to successfully complete an Enhanced Security Clearance
  • Ability to successfully pass a reference check
  • Current Emergency First Aid Certificate
  • Valid driver’s license
  • Proof of Covid-19 Vaccination
  • Completed TB Screening
  • Permanent Residency or Canadian Citizenship

COMPENSATION

The starting wage is $31.12 per hour plus benefits.

Important: Candidates must be legally authorized to work in Canada, observe their weekly hours limit if under a temporary or study visa, and provide proof of eligibility if selected for the role.
